Value Proposition

CT angiograms of the brain are commonly performed for assessment of the intracranial arteries. Accurately identifying intracranial saccular cerebral aneurysms can be challenging, especially in practices with high volumes and with non-specialized interpreting radiologists. An AI algorithm may assist with increasing the accuracy of diagnosis. Prior iterations of conventional CAD models to detect cerebral aneurysms have been limited by high false positive results. Currently published deep learning methods have also shown that aneurysm detection algorithms currently have poor specificity.

Narrative(s)

A 60-year-old female presents to the emergency department with a severe headache. A non-contrast head CT demonstrates acute subarachnoid hemorrhage (SAH) within the basilar cisterns and scattered along the cerebral convexities. A contrast enhanced CT angiogram or a non-contrast MR angiogram is urgently performed to determine the underlying etiology of the SAH.

Workflow Description

The axial source CTA  (0.5 to 1 mm slice thickness), 3 plane maximum intensity projections, and 3D volume rendered reconstructions of the circle of Willis (if available) are exported in DICOM format to the AI server/algorithm and assessed. Any blind ending vascular protrusion projecting from the intracranial vasculature or focal dilatation of an intracranial artery is detected, annotated, and displayed for review by the interpreting radiologist.

Additional parameters can be extracted (for e.g., presence of daughter bleb, internal thrombus, wall thickening/enhancement, internal flow dynamics on 4D phase contrast MRA) to estimate the risk of rupture of the aneurysm.
